A copy of the W-9 will be mailed to you but is also available on-line at http://www.irs.gov/pub/irs-pdf/fw9.pdf.

If you have not yet filled out this form please do so and attach it to your completed winner release form. Mail both forms to Fisher Radio Promotions, 140th 4th Ave W, Suite 340, Seattle, WA 98109. This address is also listed at the bottom of the congratulations letter.

If you have already filled out a 2006 W-9 form for Fisher Broadcasting, you can mail, email or fax your signed winner release form to the Radio Promotions Department. Both our fax number and e-mail addresses are listed in the congratulations letter.

Once all the necessary paperwork is received by the promotions department, you prize will be mailed to you at the address listed on your winner release form.

If you choose to pick-up your prize at Fisher Plaza, you must make an appointment with a representative from promotions. Your prize will be unavailable to you without an appointment.

*A reminder: You have two weeks from the date of your win to have your paperwork in or an appointment set for pick-up. If we do not receive your paperwork within these two weeks your prize will be automatically forfeited.
